Minnesota investigators have uncovered evidence of neglect at a Thief River Falls assisted living facility. State health officials in December 2018 visited the Valley Home assisted living facility to investigate an allegation of maltreatment. A client at the facility had complained of being short of breath, wheezing, and not feeling well. A nurse checked vital signs, wher blood oxygen saturation level was at 69 percent, well below the accepted threshold. But the staff member failed to timely inform anyone of the “significan change in health status”. The client was found about an hour later unresponsive and with only a slight pulse. Medical professionals couldn’t resusitate the client.
